May 1st Clinic for 2021-22 Athletic Physicals

Physical Form

All athletes MUST have a current physical on file with the Athletic Department before participating in any practice or try-out.

For the 2020-21 school year, MHSAA has waived physicals for students in any grade who received a valid physical for sports during the 2019-20 school year (one completed on or after April 15, 2019). These students and their parents/guardians will be required to complete the two page MHSAA annual Health Questionnaire. Visit the Athletic Office or click here for the questionnaire. 

Students who did not receive a valid physical for sports during the 2019-20 school year will need to have a physical completed on or After April 15, 2020. These students and their parents/guardians will be required to complete the two page MHSAA annual Health Questionnaire. Participation Fees

  High School: $20 first sport; $10 each additional sport.

Middle School: $10 first sport; $5 each additional sport.

These fees are payable to Sturgis Athletic Department and are required to be paid before first contest or game. Athletic fees and passes can be purchased using E Funds.


Athletic Insurance

All Sturgis Public Schools students must have medical insurance to participate in SPS athletics. Each student-athlete must provide proof of insurance ID number prior to trying out for any SPS athletic team. SPS does not offer supplemental insurance to student-athletes.

Click here for contact information to purchase insurance from an independent agency. The SPS Athletic Office and district will not be involved in any insurance claims involving the independent agency. 


Students Attending Athletic Events
  • K - 8 students must be accompanied by an adult when attending  SPS athletic events.
  • The adults must remain at the event with the students they are accompanying. If the adult leaves the student must leave with them. Any unaccompanied student will be removed from the venue.
  • Students must sit in the bleachers unless going to the restroom or concession stand.
  • Any rude/rowdy behavior by a student will result in dismissal from the event.

Athletic Antigen Testing Schedule


The MDHHS and the MHSAA has mandated that all student athletes ages 13-19 must complete an antigen test for COVID-19 on a weekly basis (during the Spring Sports Season). Testing for Sturgis Middle School & Sturgis High School student athletes will take place every Sunday evening in the Sturgis High School Auxiliary Gym per the schedule below:
